Requesting Your Health Care Records

You have the right to request a copy of your mental healthcare records. To request records, please follow these guidelines:

  • Submit a request in writing.

  • Include your name, the client’s name (i.e., minors/dependents), the client’s date of birth, requestor’s contact information including address, phone number, and email.
